In consideration of signing this agreement, I hereby for my heirs, executors assigns, administrators, and myself waive and release any and all rights, claims, and causes of action against Best Buddies, the organizers, sponsors, and any others connected with the Bank of America Chicago Marathon, their representatives, and successors, as a result of my involvement with said event. I verify that I am medically able to participate and assume all risks of participating in this event. Further, I hereby grant full permission to any and all of the foregoing to use any photographs, videotapes, motion pictures, recordings, or other record of this event for any lawful purpose, including commercial advertising. I have read and fully understand the foregoing and certify and represent that the information provided is true. I also agree and acknowledge that Best Buddies is authorized to change my credit card (on the respective "Charge Date" indicated below) for any balances due on my fundraising commitment, unless I notify Best Buddies in writing within seven (7) days of my registration of my desire to withdraw from the Best Buddies Friendship Team. The Charge Date for my remaining pledge balance due will be Friday, October 9, 2009.
